The course discusses 60 years history of blowout incidents around the world, detailing causes that led to the blowout and the Lessons learned from these incidents. Analyzing the incidents through the use of Ishikawa diagram. The objective of the course is to delve into the effect of human behavior, mainly the onsite decision makers, affecting the outcome of the incident.

Learning Objectives:
At the end of the course delegates will be able to:
  • Understand the influence of human in the initial phase prior to well becoming out of control as well as during the blow out development. Recognizing Human intervention throughout the lifecycle of drilling operations, starting with hazard identification (Hazid) and performing Hazop. Be aware of “on the fly” modification of procedures in response to real time well conditions to safe guard the well
  • To appreciate the consequences of a blow out that may lead to harm rig personnel, equipment, reputation of the company and the environment

  • Overview of blowouts in the last 60 years.
  • Human behaviour and its consequence towards blowout incidents.
  • Detail analysis of 2 recent major blowout events, the Macondo and the Montara blowouts.
  • Discussion on other blow out events where human inaction, interference and wrong judgement resulted in the event occurring.
  • Analysing root causes by use of the Ishikawa diagram.
  • Lessons learned.
